Salary Slip Format in India: Form V Explained (2026)

Published

Under the Code on Wages (Central) Rules, 2026, every employer must give employees a wage slip in Form V, on paper or electronically, on or before the day wages are paid. Form V lists the employee's details, UAN, wage period, basic, DA and allowances, attendance, overtime, gross wages, PF, ESI and other deductions, and the net wages paid.

What the law says

  • The Code on Wages, 2019 (section 50(3)) requires employers to issue wage slips.
  • Rule 52 of the Code on Wages (Central) Rules, 2026 sets the form: Form V, issued electronically or in physical form, on or before payment of wages.
  • These are the Central rules. States notify their own rules for establishments under them, so check your state's rules too.

Common mistakes

  • Issuing the slip after wages are paid. Rule 52 says on or before payment.
  • Leaving out UAN or the wage period.
  • Showing only a total under deductions. Form V lists PF, ESI and others separately.
  • Gross minus deductions not matching the net wages actually paid.
